Appointment & New Patient Information

To make an appointment please contact our office at 330-869-8530. We currently do not offer online appointment booking.

For our new patients we have provided the following download of our New Patient Paperwork

At your establishing appointment please arrive with your completed paperwork 25 minutes prior to your appointment with the following items:

  • Driver's license
  • Up to date insurance card and applicable copay
  • All medications you are currently taking (in their bottles) including any vitamins or supplements

Should you choose not to fill out your paperwork prior to your appointment you are welcome to fill it out at our office. Please arrive 45 minutes early if your paperwork is not completed.

Do you accept my insurance plan?
We do accept most insurances, however it is always best for the patient to call their particular plan prior to service to make sure we are listed as a provider.

Who do I call for billing issues?
Please contact Akron General Partners Physicians Group at 330-344-7648. They can answer your questions. They will direct you to the next step in fixing any issues.

Why does my bill come from Akron General?
In 2012 the physicians became affiliated with Partners Physicians Group. They combine a wide array of primary care groups as well as specialists. Together we are making a stronger, more complete line of healthcare providers solely based on superior patient care.

Can I still pay my balance at your office?
Yes, you can pay your balance at our office. To make it more convenient for the patient you can also address any outstanding balance you may have with other Partners Physicians Group doctors at our office.

Do you accept credit cards?
Yes, we accept all major credit cards. At this time we do NOT accepts Care Credit.

Do you have a patient portal that I can access?

Yes, we do! Please contact our office today.  Use of the patient portal will give you information at your leisure regarding your health and healthcare.
